Nancy had herself locked up in a prison although she was the one ho had been assaulted by Mr. Stovall, a Ba ptist deacon, who Nancy claimed to be the one responsible her pregnancy. She was a shamed of facing her husband because of that. Mr. Stovall knocked her to an extent of removing some of her teeth instantly and later taken as the culprit in the whole encounter. The fact that Nancy was a black American worsened her situation since she could not get fair treatment; racialism caused most of the suffering experienced in the story by Nancy. 1.2 Setting The setting of the story is at the Southern states where racial segregation had taken root with the inclusion of Jim Crow rules that discriminated the black Americans as inferior and majorly owned as slaves to work in the plantations. The constant repletion by Nancy that I am a nigger showed her resentment for being a black in a society that discriminated against the blacks. The use of orders by Mr. and Mrs. Compson when referring to Nancy shows the level at which they had assumed her to be as per the order of that time. In one instance Que ntin is asked to head to the kitchen and order Nancy that she can go home now; this is a clear indication that the physical possibility to leave is only granted by Mr. Compson without it she can not leave; she obviously did not have the right to leave before being ordered to do so. This displays the relation between Nancy and Mr. Compson as one of a slave and a master respectively (Fitzgerald and Harris, 1987). 1.3.1 Segregation The unequal relationship between the whites and the Black Americans is broadly exhibited in Nancy’s reproaching of Mr. Stovall, the deacon, specifically for not paying her the service rendered three times, for getting intimate with her. Distal Radius Fractures (DRF) Pain Management Explain how pathological processes influence physiotherapy management for a patient with a fracture of the lower end of radius. Distal radius fractures (DRF) account for 16% of fractures seen in accident and emergency. (Tosti 2011) They are often caused by a fall on an outstretched hand, and as the risk of falling and osteoporosis increases with age, elderly patients have a higher risk of DRF. DRF are described as Colles’ fractures (with dorsal angulation) or Smiths fractures (with volar angulation), and treatment varies with fracture type, age of patient and presenting symptoms. Many fractures are reduced under anaesthesia and immobilised in a plaster cast from just below the elbow to the proximal crease of the palm (Alsop 2013). During bone healing, immobilisation ensures bone ends remain aligned and reduces the risk of mal-union. Immediately after a fracture, the local bone tissue becomes necrotic, and is resorbed by osteoclasts. A fracture haematoma forms and osteoblasts produce calcium hydroxyapatite crystals which are laid down on the bone matrix, forming callus (Drake 2010). Callus is visible on x-ray at 6 weeks, which is typically when the immobilisation stage ends. During immobilisation, patients’ clinical priorities are pain management through medication, swelling reduction and prevention of secondary stiffness and muscle wastage in joints above and below the fracture. To decrease levels of exudate in tissues and aid lymphatic drainage, elevation and compression are the main physiotherapy treatments (Cheing 2005). Stretching exercises for the elbow, shoulder, metacarpal phalangeal joints and inter phalangeal joints on the affected side help maintain range of motion (ROM), and strengthening exercises for muscles of the shoulder, elbow and fingers can reduce muscle atrophy. Physiotherapists’ can provide diet education, explaining that the supplementation of vitamin D, calcium, magnesium and vitamin K will aid bone healing (Price 2012). Vitamin C is shown to improve â€Å"mechanical and histological parameters of fracture repair† in a study with rats (Gaston 2007), and to induce osteoblast differentiation, which play an impor tant role in bone healing (Carinci 2005).When the plaster is removed, skin can be flaky, thin and over sensitised. Physiotherapists can explain the importance of gentle washing and moisturising and can perform desensitising treatment if required. Due to their knowledge of fracture pathophysiology, physiotherapists can advise patients on how to protect their wrist, for example, not to lift a full kettle but to continue with functional tasks such as washing dishes. After immobilisation, an important symptom is pain, affecting the patient’s ability to perform functional activities. Pain or fear of pain can impair treatment, as the patient may be nervous to do their prescribed exercises. Effective pain management in the form of paracetamol and ibuprofen, and explanation that a dull aching pain is demonstrative of bone healing may help reduce patient anxiety. Measuring pain allows physiotherapists’ to provide outcome measures and to tailor treatment to patients’ individual needs. Self-reported measures, such as the visual analogue scale, are the gold standard for measuring pain intensity, location, quality and temporal variation (Jones 2013). Nociception from DRF occurs when the sensory receptors at nerve endings in the periosteum are stimulated by noxious insults that are produced through inflammation (DeLisa 2005). An action potential is carried to the dorsal horn of the spinal cord where the pain signal is sent to the brain . As pain is transmitted via the dorsal horn, physiotherapists use modalities that use the pain-gate theory to reduce patient’s discomfort. This theory suggests there is a gating mechanism in the dorsal horn, small nociceptors that carry pain facilitate the gate, but larger mechanoreceptor fibres inhibit the gate. When physiotherapists stimulate mechanoreceptors, the gate is inhibited and pain signals transmitted to the brain are reduced (Moayedi 2012). An example of this is accessory mobilisations, where the physiotherapist recreates athrokinematic movements to stimulate mechanoreceptors, inhibiting nociception. In a DRF, all athrokinematic movements can be used at grade one and two to stimulate mechanoreceptors. Massage uses the pain-gate theory, therefore alongside the physiological effects of massage, such as increasing blood flow and lymphatic drainage, massage stimulates the mechanoreceptors that inhibit the gate, inhibiting pain signals. Stiffness can be caused by a variety of aetiologies. If the fracture involves articular surfaces, blood entering the joint can leave fibrin residue causing fibrous adhesions between the two synovial membranes (Hamblen 2007). This decreases the congruency of the surfaces, therefore decreasing ROM. More commonly, peri-articular adhesions, caused by collections of exudate, reduce the resilience of ligaments and reduces muscles free gliding abilities, causing stiffness. (Hamblen 2007). If the patient has undergone open reduction surgery, scar tissue can cause adhesion of local muscles and tendons, reducing ROM. Proprioceptive neuromuscular facilitation (PNF) is a modality used to treat decreased ROM. PNF uses the proprioceptive stimulation of muscle groups, using voluntary muscle contractions alongside stretching to reduce the reflexive aspect of muscular contraction (Mahieu 2008). Using maximal muscle contraction enables maximum relaxation, which increases stretch efficacy. By using this technique on physiological movements of the wrist, the adhesions are broken down allowing fluent movement. Simple home stretching exercises can be prescribed, to ensure that soft tissues are stretched frequently to reduce stiffness. As well as treating pain, mobilisations are used to decrease stiffness. For stiffness, both accessory and physiological passive mobilisations can be used to increase ROM. When treating stiffness, grade three and four mobilisations taken to the end of range are used, which break down peri-articular adhesions and allow synovial sweep, creating even lubrication and reducing fricti on. After pain, swelling and ROM have been addressed, strengthening excercises are incorporated into treatment to reduce muscle atrophy caused by immobilisation (Powers 2004). Strengthening excercises help to regain muscle mass and strength, by causing neural adaptions, decreasing inhibitory feedback allowing stronger contractions. Stronger contraction is also caused by muscle hypertrophy, where myocytes enlarge, increasing actin and myosin concentration. Excercises should get increasingly more challenging until functional movement is achieved. All excercises should be aimed at functional goals specific to the patient, increasing motivation and also establishing expectations of both the physiotherapist and the patient. Due to NHS cuts, physiotherapists can not see patients as frequently as desired, therefore modalities such as massage and PNF cannot be fully effective. It is therefore important for the physiotherapist to increase motivation for home excercises through explanations of the ir importance and effects . As the most common cause of a DRF is falling on an outstretched hand, physiotherapy falls prevention programmes including gait re-education,walking aids and balance exercises, can reduce the risk of DRF. These programmes have been â€Å"associated with a significantly lower risk of fractures† (El-Khoury 2013), demonstrating that prevention is the most effective physiotherapy management for both patient and physiotherapist. The Death Penalty and the Eighth Amendment   Ã‚  Ã‚  Ã‚   Is the death penalty consistent with the Eighth Amendment's prohibition against the imposition of cruel and unusual punishments? This essay will address this question and present a short history of the death penalty in America.    The Supreme Court considered particular applications of the death penalty in the 1940s and 1950s. In each case it upheld the state's action without addressing the larger issue of the death penalty's constitutionality. In the 1960s, the NAACP Legal Defense Fund, led by Professor Anthony Amsterdam mounted a full-scale attack on the death penalty. Adopting a "moratorium strategy," the LDF succeeded in blocking all executions for five years, creating a "death-row logjam."    In Furman v Georgia in 1972, the Court invalidated all then-existing death penalty laws based on the inherent arbitrariness of their application. Most observers at the time concluded that there would never again be an execution in the United States. They were wrong. In 1976, in Gregg v. Georgia, the Court upheld Georgia's new capital-sentencing procedures, concluding that they had sufficiently reduced the problem of arbitrary and capricious imposition of death associated with earlier statutes.    The Court continued to face questions concerning the application of the death penalty: to non-murderers, to minors, to mentally disabled prisoners, to racial minorities. One such case is McCleskey v. Kemp, a challenge based on a study that showed murderers of white victims were far more likely to be sentenced to death than murderers of black victims.    The Eighth Amendment says: Excessive bail shall not be required, nor excessive fines imposed, nor cruel and unusual punishments inflicted (Constitution).   How does it square with the actual facts of capital punishment: 1. The average inmate put to death in 1998 spent ten years and ten months on death row. 2. In 1998, 285 people were sentenced to death. 3. At the close of 1999, Texas has the most prisoners on death row (39), followed by California (31), and Alabama and Florida (25 each). 4. Over 3,500 prisoners are currently on death row. 5. Thirty-nine states have death penalty laws on their books.    One of the most eloquent attacks on the death penalty ever delivered in an American courtroom came in the Leopold & Loeb Case in 1924. Clarence Darrow's eloquence is often credited with saving the lives of two confessed teenaged murderers.

What information about diversity in the United States has helped you better understand or relate to others in ways that you may not have in the past? Diversity in the united states in not something new here but in fact spans back many generations. Diversity is everywhere and no one can truly get away from it, but by accepting it a person can learn how to lead a better fuller life with variety of understanding for the people that come in contact with. I feel that the biggest thing I learned in this call about diversity is not that there is diversity but that there is a lot of work to be done to farther the peace action between the groups of people knowing this make me realize that the us is not yet a unity but is trying and gives me more will to make my voice heard that we need to start accepting one another. †¢Have you learned something new about your own racial, ethnic, or cultural history? My own racial, ethnic, or cultural history is a great big mess of origins, religions, and culture but this class made me realize that one I am not alone in time case and that I too as a person have boundaries that hold me back. For instance there is a glass ceiling which is â€Å"the barrier that blocks the promotion of a qualified worker because of gender or minority membership† (ch.3, pg. 78) this is not a barrier of race or religion or culture its based on gender and I being a women hinders my ability to progress in some areas of the working world. This is some thin that women from not just the United States have been fighting for but the United States I see ha made great strides to make things more equal and fair. †¢Trends in immigration will continue to shape the demographics of the United States. What will the U.S. population look like in the year 2050? Why do you think so? With the raising population factor and the immigrant that keep flowing in will make the melting pot a whole lot bigger making some big changes like the minorities becoming majorities and vies versa. In an article on CNN U.S. website had this to say â€Å"There’s going to be a very diverse minority population with large amounts of interracial marriage. What we’ll see is that the Anglo-Saxon population will no longer be the majority. I believe the mixed-race population will grow the fastest because compared to the past, interracial relationships are increasingly tolerated and considered OK by a large percent of the younger generations. It’s no longer strange to see a black and white couple at a restaurant together today. Yet someone visiting from the 1960s would be shocked.† (Kotkin,2010) this just comes to show you that there are a lot of changes in the works for this country. †¢What challenges does the United States face due to the diversity of its people? The United States is by far more diverse then other countries, on a larger scale and because of that many parts of this country has states with cities that are unintentionally segregated by race and culture. This type of living is ok but it also make be shy away from each other and not interact , thus giving way to discrimination and prejudice due to the lack of understanding of one another. With any type of misunderstanding can bring new problems to the table and this only made it hard to for the government to mandate a resolve for it, but if we could start mingling more intergrading more that misunderstand will start to disappear lessening the amount of discrimination in the country. †¢What are the benefits of such a diverse society? The benefits of such a diverse society in the United States is an amazing one and growing every day. The United States is a melting pot of races, cultures, religions, and sexual preferences in many forms and this all make for a great example that we can all get along. What I’m trying to say is that we are cutting out the misunderstanding between the categories of people and back down the boundaries that were set to control. We are not a country at war we are a country that is showing that it is possible to live in peace without letting discrimination and prejudice turn us against one another. In other wording the U.S. is showing that it’s ok to be one and to stand together and in time I think we will. †¢How can we foster a climate of acceptance and cultural pluralism in the United States? Pluralism is a state in which racial and ethnic categories, though distinct, have equal social standing. (Macionis, 2012 pg.84) So in stating what it is we are looking to foster gives us a better idea of what to expect if we do foster a climate of acceptance and cultural pluralism in the United States. Though to get there we need to teach, I mean really teach each other and about one another in every way form what we eat to how we celebrate traditions. We need to get everyone involved as much as possible, which means in schools, jobs, and community. By doing all of this we will have a better understanding of one another which well level out the minorities with the majorities and make acceptance easier all around. †¢In what ways does the media perpetuate stereotyping and prejudice? Provide examples to support your assertion. The media I feel perpetuates stereotyping and prejudice by ways of take on old way and making them fix into the new. This is still stereotyping, it is still racism, it is still discrimination and it is wrong, wrong because the media is not trying to help resolve issue of this topic but in an effect helping to create it. In this day and age that media is watched more than ever and it is teaching that it is ok to by this way or view people in such a light which can hinder the process of pluralism. In an article I found stated the following â€Å"Bradley Gorham researched this problem in the article â€Å"News media’s relationship with stereotyping† (2006). Gorham maintains that, â€Å"stereotypic images of Blacks persist in the dominant media†¦as a result; stereotypes are perpetuated within the culture in subtle, yet highly effectual ways† (Gorham, 2006, 289). In addition, a 1991 study conducted by Northwestern University on the images of races in local TV news programs, concluded that television stations serve to promote what researchers deem ‘modern racism’ or â€Å"the continued, though muted, antagonism between races† and that in their efforts to overcome racism, stations have actually served to worsen â€Å"racial hostilities† (Bowling, 2004).†(Henry, 2007) †¢In what ways does the media help foster appreciation for diversity? Provide examples to support your assertion. Though the media perpetuate some stereotyping and prejudice, it also does a lot of good. In the media you can find hundreds of advertisements that are out there to help the progression of unity and pluralism, but also television shows and documentaries that have made huge strides to fight for the right of everyone and show real depictions of how much easies life could be if we just all got along. A great film called Life in a Day, which is a film that is compiled of thousands of people throughout the world on one single day, the 24th of July. The Director Macdonald said that the film focused on a single day â€Å"because a day is the basic temporal building block of human life—wherever you are,†(Watercutter, 2011) this is a view of how truly alike we all are and its film like this that make the media help foster appreciation for diversity. †¢How might individuals and the United States work together to reduce prejudice and increase appreciation for diversity? We a people need to make a point to make an act of kindness every day towards those that need help and teach our children our adults of the future to be blind of race, religion, culture, and anything else and learn to fight for one another in every way possible. I feel that one good act of kindness or one good stand of what is right can create a beautiful butterfly effect of future unity and pluralism. Other things that might work is to mandate regulations of team working in company’s and schools so to in a sense force unity in the work place making those who normally would not work together to work side by side and see firsthand the they are on different from one another. †¢How might you change your own behaviors to be more inclusive and pluralistic? My behaviors to be more inclusive and pluralistic has started many years ago when I was 12, our cool had a school meeting to discuss the issue our school was having with discrimination and exclusion of students into due to their differences. The principal ask if any of us had any ideas that we felt could and I stated that instead of selling the extra land the school was trying to sell and make it into a team working ropes course and it was voted on and created and still used today. I have always found ways to help others get along and or view things difference, I make a point to set an example for my child and any one the needs an example of how to be a good human not just a person. In this day and age there still is many issue related to discrimination and prejudice that need to be resolve and yes it will take time but one step can lead to many running towards the better good of mankind ,unity, and pluralism of the country and maybe someday the world.
